Normally mooring goes well, but as time goes by, the level of safety slowly declines. Maybe you lose concentration, maybe you slacken your procedures just a little bit, maybe you get a little complacent. And then it suddenly happens – not because of one factor but because of a number of interacting factors.

• Ship’s safety culture
• Weather
Preventing accidents is about reducing the risks of those factors. The only parameter that is hard to overcome in this respect is the weather.

dsutra, Don't just rely on the auto search which happens when you are typing the title for the new release.
Rule 8.1: Before release creation, check that there is no the same release on the tracker uploaded (use the search or enter the title in the release creation form).
It is the duty of the releaser to check and verify that the book is not already present. Search is much improved now.
1) Do a manual search for the title (Less words & key words...avoid punctuations and hyphens etc.) e.g. Mooring alone would be sufficient. You can add more words to filter the search.
2) Google search within the tracker. Avoid copy pasting the title and write manually without punctuations.
